Please do not send SSNs, dates of birth, bank information, medical records, X-rays, insurance cards, or other sensitive documents by email.

For a correction: the page URL and the source that appears more current.
For a clinic listing update: the public page showing the payment option.
For a partnership question: the clinic, market, and type of dental cases handled.
For a form issue: the page, browser, and approximate time of the submission.

If you see a financing term, lender detail, or cost figure that appears outdated, send the page URL and the source you believe is more current.
